If the information you enter does not match the records in our system, you will receive an error message. Check the information requested to ensure you entered it accurately. If the information entered is correct, and if you still have problems to login, please contact CityHealth at 510-984-2489 (or other health care provider where you collected your specimen) to update your name and date of birth.

Cannot download my report from the internet.

Many problems are caused by external programs downloading something. Clear internet data and cache. If the issues are not fixed, please try a different browser, or login on the computer instead of your phone.

What is Barcode?

Barcode is the number (starting with a letter following with 6-digit number) located at the low portion of the barcode label putting on your collection tube. By using barcode, the system can uniquely identify your test results and report. For your convenience, we usually stick an extra copy of the sample barcode on your handout.

Alternative way to download test report with sample Barcode.

You can visit https://covid19-portal.predicine.com/download_report to download your test report if you know the sample Barcode. For security purpose, email or text message verification is required before you are able to download your report.

Information on my report is incorrect?

If you find any incorrect patient information on your report, please contact CityHealth Urgent Care at 510-984-2489 (or other health care provider where you collected your specimen) for error correction.

Why I can’t receive patient report by email?

Directly send patient report through email may violate the HIPPA (Health Insurance Portability and Accountability Act) regulation.
